Symptom Surveillance
Problem: Most symptom information is documented in the HPI section of the clinical note, which is usually done in a free text mode. This makes the process for retrieving documents based on symptom data or extracting specific symptom data a time-consuming, labor-intensive task.
Solution: L&C’s technology automatically semantically indexes the free text in clinical notes creating the ability to automatically retrieve patient records based on symptoms, or it can extract specific symptom data to populate a spreadsheet or database
Clinical Decision support
Problem: Often a physician needs to refer to the current medical literature related to the problem being treated at the point of care. However, doing a Medline search is difficult, time-consuming and inconvenient.
Solution: Once triggered, TeSSI automatically extracts the most semantically relevant clinical code and transmits it to the provider’s content source for immediate retrieval of the most relevant current literature or article for clinical decision support.

Patient Education
Problem: Distributing clinical information related to a current procedure or problem of a patient is time-consuming and difficult.
Solution: L&C’s NLP technology will identify and extract the appropriate concept and send to the provider organization’s consumer content data store for retrieval of the most relevant article(s) back to the point of care, delivery via secure messaging or e-mail or to the patient’s consumer health record.
Clinical Trials Recruitment
Problem: Too many patients who actually qualify for a clinical trial are never identified and located because much of the qualifying symptom information is buried in the free text portion of the clinical notes. There are too many missed opportunities.
Solution: L&C’s NLP technology automatically processes the free text in the clinical notes and finds the records of the patients with the right combination of symptoms and other variables required for the clinical trial. All qualifying patients are found automatically.

Patient Safety
Problem: Patient safety is significantly compromised by medication errors.
Solution: L&C offers a software plug-in that automatically captures and structures prescription information from order entry systems, discharge summaries, and office notes and links to drug databases and EMRs to look for contra-indications based on what drugs the patient is already taking.
User Adoption of the EMR
Problem: Physicians resist the notion of structured data input for HPI section of the EMR because of the degree of difficulty in doing it, the lack of flexibility of expression and the interruption to the workflow; hence, low user adoption.
Solution: Let physicians continue to dictate or type the HPI portion of the medical record, and L&C’s technology will automatically identify the medical terms within the text, look up the concepts behind these terms, and place the concepts and codes such as ICD, SNOMED, Medcin, etc. that are linked to the concepts into an index. These indexed terms can then be used for a number of processes such as coding, list management, symptom surveillance, decision support and data extraction.
