CONSULTATIONS: None. MEDICATIONS ON ADMISSION: Lipitor 40 mg 1 p.o. q.d., Coumadin 1.25 mg 1 p.o. q.d., Toprol XL 50 mg 1 p.o. q.d., Isordil 40 mg 1 p.o. b.i.d., Persantine 75 mg 1 p.o. t.i.d., Norvasc 50 mg 1 p.o. q.d., Prilosec 20 mg 1 p.o. q.d., prednisone 8 mg 1 p.o. q.d., Elavil 20 mg 1 p.o. q.h.s., Colestid 2 gm 1 p.o. b.i.d., Zestril 5 mg 1 p.o. q.d., sublingual nitroglycerin taken p.r.n. chest pain. ALLERGIES: The patient is allergic to codeine which causes pruritus. SOCIAL HISTORY: The patient is married and is currently retired. FAMILY HISTORY: Noncontributory. PHYSICAL EXAMINATION: The patient is an obese, white male in no apparent distress. Pulse 85, respirations 20, blood pressure 138/84. Temperature is afebrile. HEENT: Pupils are equal, round and reactive to light. Extraocular movements intact. Nasopharynx and oropharynx are clear. NECK: Supple without lymphadenopathy, JVD, bruits or thyromegaly. CARDIOVASCULAR: Heart sounds are irregularly irregular without murmurs, rubs or gallops. PULMONARY: Revealed bibasilar scattered rales. ABDOMEN: Soft, nontender with good bowel sounds. There was mild lower quadrant tenderness noted. EXTREMITIES: Without clubbing, cyanosis or edema. Pulses were 2+ and equal bilaterally. There were no femoral artery bruits present. NEUROLOGICAL: The patient was alert and oriented x 4. There were no motor or sensory abnormalities. Deep tendon reflexes were 2+ and equal bilaterally in the upper and lower extremities. RECTAL: Heme negative.
